Assisted Living & Senior Independent Living Care

When patients are too old or sick to come to us then we bring our healthcare services to the patient. In this aspect, we are known for our geriatric care services at several Assisted Living Centers. In addition we also service HUD Independent Senior Apartment buildings where we bring vital ambulatory health services to income-disadvantaged seniors who are home-bound or otherwise unable to travel to their doctors offices.

The HomeAtLast™-HouseCall Program

Yes! We do House Calls! Our HomeAtLast™ housecall program is geographically restricted to the Reston-Herndon area only and is available to Medicare eligible seniors who have just been discharged from the hospital and are too well to remain in the hospital but not well enough to travel to the doctor’s office. This is a short-term program where-in we visit the patient at his or her home. The program is delivered in conjunction with a Home Health agency who work closely with our clinic to coordinate care delivery at every stage.

And better control makes it easy for participants to manage their disease and stay in the program longer.

Is it effective?

Since 2005, the program's participants have been able to meet or exceed the program's goals. And they have been able to keep their disease in control long after completing the program. Almost 80% of the programs participants were able to do so without medication. It is not at all unusual for patients to enroll while on insulin and within 3-6 months achieve levels of control that enable them to get off their insulin. How much does it cost?

Other than your insurance co-payments for every visit, the program itself is absolutely free. We do not charge patients separately for the program. If you have health insurance that we accept, then the only costs to you are your co-payments for every visit and any deductibles that your plan may impose. And patients without conventional health insurance can benefit from our direct-pay program, which is designed to ensure that this program benefits the maximum number of people.

We have an affordable self-pay program for people without conventional insurance. We intentionally keep the rates low to make it affordable to the maximum number of people possible - including temporary overseas travelers visiting the USA.
